Before you begin
This is a 50-question sample drawn from two larger self-assessment tools. It is a high-level preparedness check, not a comprehensive audit and not an inspection. It covers only a small portion of what a surveyor or an authority having jurisdiction reviews. A high score does not mean your center is survey-ready or that your building is code-compliant, and it does not predict a deficiency-free survey. This is not legal, regulatory, accreditation, engineering, or fire-code advice. Use it only as a starting point, and always work from the current regulations, your accreditation organization’s standards, your authority having jurisdiction, and your own governing-body-approved policies.
How to answer
Mark each item Yes (fully in place), Partial (in progress or incomplete), No (not in place), or N/A (does not apply to your center). N/A items are left out of your score, so a center that does not have a piece of equipment is not penalized for it. Yes counts as 1, Partial as one-half, No as zero.
Where a question mentions a frequency that the regulations do not fix, the wording follows common survey expectations rather than a stated rule. Check those against your own governing-body-approved policies.

About these questions and the fine print
